IF YOU WANT YOUR CHILD TO HAVE MEDICATION ON FIELD TRIPS:
 
MCPS Policy requires that the proper forms are completed and the medication collected prior to all extended day field trips to ensure safety.  A designated member of the staff will carry the medication, along with the appropriate forms and administer the medication as specified.

Medication and permission forms must be brought to the school 3 days prior to the field trip
**If we do not receive the medication and permission forms by this time, your child will not receive medication on the trip.


Medication already in the school clinic is sent with the student on all field trips.
  • If your child already has medication and forms on file in the clinic for the current school year, it is NOT necessary to complete new forms for the field trip.

Over-the-Counter Medication:
  1. Complete one Montgomery County Medication Administration Form for each individual medication that will be administered (one form for each medication).
  2. Medication form must be signed by the parent/guardian.
   3.   Parent must bring the exact amount of medication needed during the trip in its original container to the                    school.  Travel size container is appreciated.

Prescription Medication:
  1. Complete one MCPS Medication Administration Form for each individual medication that will be administered (one form for each medication).
  2. Medication form must be signed by both the parent/guardian and the physician.
    3.  For your convenience, you may sign the form and ask your physician to complete the remainder of the form and fax it          to the school at (540) 951-5808
    4.  Parent must bring the exact amount of medication needed during the trip in its original container to the                    school.

Medication Permission Forms are available:
  • Online on BMS website under “For Parents” tab then choose “Nurse” and click on Forms
  • In school clinic
  • In school office

*Please note the 8th Grade DC field trip requires that a Field Trip Health Information form must be completed by all students participating on the trip.  If your child requires medication on the field trip, the Medication Administration Permission Form must also be completed for each individual medication.
