IMMUNIZATIONS
All risingth 7th graders are required to receive a Tdap booster, Meningococcal and HPV vaccines before the 1st day of school. This is in addition to the immunizations received prior to kindergarten.

REQUIRED IMMUNIZATIONS for Entering 7th Grade
Tdap Booster- 1 dose
Meningococcal -1 dose
HPV -1 dose (documentation is not required)
Required prior to kindergarten
DPT- 4 doses
Hepatitis B- 3 doses
MMR- 2 doses
Polio- 4 doses
Varicella- 2 doses
Hepatitis A -2 dose
